The COACH Program^®

The COACH Program is an innovative coaching program for people with chronic disease. The COACH Program trains people with coronary heart disease or other related chronic diseases to manage their cardiovascular risk factors while working with their usual doctor(s).

It is the world's only evidence-based program of coaching for the prevention of chronic disease and has been adopted as standard care in the public and private health systems across Australia and has expanded to Europe.

The COACH Program was developed to bridge the gap in management of cardiovascular disease between what is known from published evidence and what is actually practised. Despite compelling evidence for aggressive risk factor management in the ongoing treatment of cardiac disease, there is evidence that only a minority of people are achieving target levels for risk factors such as LDL-cholesterol and blood pressure. Not achieving these targets increases the chance of further health problems.

With its 15 year track record The COACH Program is supported by the highest level of scientific evidence with two published randomised controlled trials which prove that The COACH Program is highly effective in reducing risk coronary risk factors (like cholesterol, blood pressure etc) in patients with cardiovascular disease. An independent study conducted by the Victorian Department of Human Services has showed that The COACH Program reduces hospital utilisation by up to 20 per cent compared to standard care alone.
